Dialectical Behavior Therapy (DBT)

Dialectical Behavior Therapy (DBT) is an empirically validated approach to behavior therapy that was originally developed to treat suicidal ideation and clients diagnosed with borderline personality disorder (Linehan, 1993). Fundamentally, the theoretical basis of DBT is the dialectical philosophy that two, ostensibly opposing elements can be true at the same time (Linehan). Regulating acceptance that these elements exists are as they need or are meant to be, with the concurrent truth that the status needs to change is an illustration of the process of dialectical philosophy.

There are some basic similarities between sandtray therapy and DBT that suggest that the integration of the two is an exciting possibility. Some examples: 1) Sandtray therapy is focused on the tray and the process creating a container, and DBT places emphasis on distress tolerance—which is arguably assisted and attained within the context of temenos; 2) As an expressive therapy, sandtray therapy looks to develop metaphorical and literal awareness and insight, and DBT has a significant focus on mindfulness; 3) Sandtray therapy seeks to create a holding environment, which is matched by DBT’s focus on maintaining an egalitarian perspective; and 4) Sandtray therapy offers the opportunity for symbolization and sublimation through the creation process and the miniature figures, and DBT places emphasis on emotional regulation—which is assisted by the therapeutic distance created through this symbolic and sublimating dynamic.

Linehan and Wilks (2015) summarize the four skills modules of DBT:

mindfulness

interpersonal effectiveness

emotional regulation

distress tolerance.
